NYS Part 100 Regulations list the Home Schooling responsibilities of both the district and the parent.

Parents are required to submit an Individualized Home Instruction Plan and Quarterly Reports. The Individualized Home Instruction Plan form provided in the link on the bottom of this page is a template that may be used, or parents may submit their own form, as long as it contains the required information.
